Scheduling An Appointment

Protocol

1. Call the desk to schedule: (503) 658-0316 (Remember! We're Pacific Standard Time).
2. A registration packet will be mailed to you. Please mail or fax the registration form as soon as possible, along with any pertinent medical history, test results or information. You may complete our online registration form for animals or people. Please call or email to establish an appointment time prior to submitting the registration form. Indicate your established appointment time in the space on the online form.
3. Call the phone consult line at your pre-arranged time.
4. After the initial consult, Dr. Starita will discuss the results of the energetic analysis and recommend direction for a return to well being. A synopsis will be sent to your primary care provider.

It is recommended that each new patient have a thorough exam by their primary health care provider at least yearly and for each current health challenge.

Consult and Energy Analysis Fee Schedule
New Patients:
We recommend an initial 60 minute appointment for people, $220.
An initial 40 minute session for animals is $150.

Rechecks for people: 30 minutes, $113.
Rechecks for animals: 20 minutes, $75.

Products and supplements, as well as laboratory tests are additional.

Payment:
We accept VISA, MC, checks or cash. Please let us know how you'll be paying when you schedule your appointment.
